cdocuments-and-settingslsteinbedesktopchanel_couturefall2009Forewarning: I’m going to toot my own horn and say that thanks to great genes from Mom, I have clear, even-toned, blemish-free skin. Only during cold, dry winters does my complexion show signs of (slight) distress, in the form of rosy cheeks and lackluster luminescence. But then this summer, in the middle of July, I spotted a spot. Small but noticeable because of its alarming redness—a pimple. Located just above the upper right corner of my mouth, where my lips turn up when I smile, this unwelcome blemish made its home. Upon further inspection, a cluster of what I’ll call “congested pores” seemed to be spreading across my forehead and dotting my chin. Feeling frantic, with magnifying mirror in one hand and phone in the other, I quickly dialed in an emergency to the only person I felt qualified to handle this situation: facialist Joanna Vargas.

I’d first heard of Vargas from a fellow editor whose mother, sister, and everyone else in her family all swore by. Hamish Bowles, our dapper Editor-at-Large, was also won over by Vargas’ touch: He’d come to the beauty department the day of the Met Costume Institute Gala one year looking for an aesthetician who met the following criteria: close in proximity to Vogue’s Times Square offices (her space is across the street from the New York Public Library), and able to thoroughly clarify the skin without the usual post-facial inflammation. Off to Vargas he’d gone, and an hour later he’d walked ready for the red carpet.

So after a flurry of emergency phone calls and E-mails, an appointment was on the books. I pushed my bangs across my forehead to hide the unsightly, and waited for the hour of my appointment to arrive. Once inside Vargas’s miniature midtown spa—nothing more than a single simple treatment room—I quickly changed into a plush terry-cloth robe and lay down on a table wrapped in the same soft white cloth. Moments later, Vargas entered the room with her own glowing, dewy skin (a sign of things to come), auburn curls neatly tied back, and took a seat at the crown of my head. Off to work she went, smoothing a coconut-cream cleanser by Eminence (that I have been addicted to ever since) over my face and sending me into a state of complete relaxation—of the eyes-closed, even-breathing variety. My bliss was interrupted, however, by a jarring aroma more likely to be found in an Italian restaurant than on my face: tomatoes and garlic. Every facialist has a secret recipe, but was this really necessary, I pleaded, terrified I would walk out smelling like a bowl of pasta. “I’m pulling out all the stops on you today,” she said in a calm, soothing voice. “Tomatoes are loaded with antioxidants, and the garlic’s antibacterial properties will protect the areas where I’ve done extractions of New York City grime.” Extractions? Because Vargas is so delicate and skilled with the tools of her trade, I barely noticed she’d been poking and prodding with otherwise menacing tweezers and lancets. She followed the tomato mask with a calming wash of chamomile water, an application of a rose-petal-and-peppermint serum, and a dollop of apricot cream. Her finishing touches: sunscreen (the mark of a good facialist) and a swipe of lip balm. Gone were the tomato and garlic; instead I smelled sweet, like apricots (Vargas stayed true to her promise). But a look in the mirror before leaving was the real reason I’ll be going back: perfect, glowing skin without a trace of the blemishes, blackheads, and overall inflammation I’d walked in with.

Joanna Vargas’ salon is tucked away in an intimate space in a non-descript office building near Bryant Park. With only two treatment rooms, Vargas gives her celebrity and fashionista clientele the personal attention they crave. The 39-year-old Latina has had her own spa, Joanna Vargas Skin Care, for three years and has been an aesthetician for 12 years.

“I thought I would do [fashion] makeup, but I didn’t care for the amount of pressure and yelling on a photo shoot,” the University of Chicago graduate says. “So I went to aesthetics school and fell in love with skincare instead.

I recently visited Joanna’s salon and sampled her signature Triple Crown Facial, which utilizes a mild electrical current known as microcurrent. Here’s what she had to say about her innovative techniques:

Hot Mama Daily: Tell me about the Triple Crown Facial.

Joanna Vargas: It’s called the Triple Crown facial ($250) because there are three components: A diamond microdermabrasion at the beginning; I don’t do any facial without that step. Then the majority of the facial is done with microcurrent, which drains excess puffiness, tightens the muscle, and restores the skin. The final part of the facial is an oxygen treatment, which is 98 percent pure oxygen, infused with a serum of green tea, vitamins and aloe.

HMD: What are the benefits of getting a facial like this as opposed to a traditional facial with extractions?

JV: I do traditional facials as well. The focus with this facial is anti-aging and the general health of the skin. Extractions are good if you have something to come out, but a lot of people don’t need extractions. I believe [the Triple Crown Facial] is like going to the gym for your face. It keeps your muscles strengthened and makes sure your skin is a healthy as it can be. It gives you a glow. I think when a woman feels healthy and like she’s doing everything she can for her body, she feels confident. And that translates into looking beautiful.

HMD: I have some hyper-pigmentation and acne scarring. What would you suggest?

JV: I do something called a VitaLight treatment ($150) that uses LED light therapy. It is a proven treatment that uses red and infrared light that corrects any skin damage and stimulates your lymphatic system. It also causes cells to produce ATP, which the cells need to maintain proper health. Pretty much all of my clients do LED treatments as well as weekly or monthly facials. LED is great for anything from acne scars and wrinkles to active acne. It really helps all skin types. For general maintenance, clients get it with their facial once a month.

HMD: What would you suggest for me in terms of maintenance?

JV: You don’t really have a lot of extractions. But you need to exfoliate more.

HMD: I use a buff puff. A lot of aestheticians tell me not to.

JV: [laughing] Okay, that’s not good. I recommend using organic products to everyone. They help restore the glow to your skin and I think the body assimilates them better than products with chemicals. After age 30, you have to step it up a bit. You can’t just use whatever product, and good products don’t have to be expensive. You want to feed your skin the right vitamins and nutrients, just like when you eat a salad with a healthy protein instead of a burger every day. For you, I would also recommend a monthly Triple Crown facial and VitaLight therapy to bring out your skin tone in general. That would pretty much keep everyone guessing about your age.

HMD: I like that.

ORGANIC, ANTI-AGING FACIALS AND SKINCARE PRODUCTS

illustrationlogo98w241hIn three weeks, I’ll be thirty-one. No longer “just turned thirty” but “in my thirties” or “thirty-something”…geesh. Don’t get me wrong, I’m loving all this wisdom that I’ve acquired in my three plus decades, and I still get carded at the club and approached by guys who are not even old enough to buy me a drink.

But, it seems like everything during adulthood requires more work – relationships, success, happiness and yes, beauty! Not only am I stacking the cupboards and refrigerator with organic treats to improve and maintain my body, but I recently went on a mission to find organic skincare treatments in the hopes of preserving a youthful glow.

Maybe it’s luck or simply fate that I stumbled upon Joanna Vargas a.k.a miracle worker. Joanna has her own boutique salon in Manhattan, has worked with numerous celebs and is known for using non-invasive, anti-aging techniques along with organic skincare products.

To be honest, I went into that salon not expecting to see any immediate results. I usually shy away from facials because of my sensitive skin and as a thirty year old black woman, I’m not exactly worried about wrinkles or fine lines at this point. All I wanted were clear pores and a routine that would help me preserve what I’m already working with.

I got their signature, anti-aging Triple Crown Facial. It’s a three step facial that begins with microdermabrasion then microcurrent and ends with pure oxygen infused with an antioxidant serum. It sounded like I was getting the works, and it was a gentle, relaxing experience. But, after Joanna applied the microcurrent on the left side of my face and asked me to take a look in the mirror. I was speechless!

There was a significant difference. My cheeks looked more defined, no puffiness around my nose and eyes and a tighter jaw line. Guess I didn’t notice the slight changes over the years, but one half of the woman in the mirror looked five years younger than the other. According to Joanna, toxins from alcohol (those damn French martinis) and lack of sleep can cause sagging muscles and inflammation.

The microdermabrasion exfoliated my dead skin and then the face muscles and skin are stimulated with microcurrent to promote collagen and elastin. It’s absolutely amazing and just one of those things that you have to experience to believe. This treatment will run you $250 to $325 but it’s a long term investment that’s worth every penny. Think of your favorite designer shoes and bags and whether they make you look younger or more beautiful.

The salon also uses and sells all organic toners, masks, exfoliators as well as other stuff for your hair, body and even home. I think this spot will be on my list of places to frequent.

980692_blogProperly caring for residents’ skin is a year-round requirement, but for many providers it becomes an especially tough challenge in summer months. The blazing sun and high temperatures can pose an even greater threat than usual to fragile, aging skin.

Just a few moments in direct sunlight can lead to a host of problems: sunburn, discomfort and the potential for subsequent skin breakdown. Higher temperatures (coupled with either excessive humidity or the dryness of arid climates) also can wreak havoc on skin integrity, experts warn.

“Aging skin is thinner, sensitive and more prone to damage, so good skin care is always important,” notes Eric Goldman, M.D., a Matrix Medical Network physician who works at Sarah Neuman Center for Healthcare and Rehabilitation in Mamaroneck, NY. “But as more people venture outdoors to enjoy the better weather, there are [additional] factors that need to be considered.”

Fortunately, keeping residents’ skin healthy and intact doesn’t require a tremendous amount of effort. With proper assessments, the right treatments and a healthy dose of staff education, providers can feel secure when their residents head outdoors to take in some fresh summer air.

Experts generally agree that many of the basic rules followed in cooler weather months will still apply.
Among the most important? Keep skin care tailored to each resident, as opposed to following a one-size-fits-all formula.

“Skin care should be individualized, based on the resident’s medical history (taking into consideration medication use, skin cancer history other dermatologic conditions, such as eczema or psoriasis),” says Sharon Brangman, M.D., president-elect of the American Geriatrics Society in New York.

Never ease up

Preventive skin and risk assessment protocols also should be followed faithfully, using tools such as the Braden Scale or Norton Scale. Risk assessments should take into consideration mobility, incontinence, sensory deficiency, and nutritional status, including conditions such as dehydration.

Although skilled nursing residents might be less likely to regularly venture outdoors, their caregivers cannot afford to become lax when it comes to summertime skin care regimens.

“In our setting, we are very proactive,” notes Darien Tully, RN, charge nurse at Lima Estates, an ACTS retirement community in Media, PA. “Every resident gets a [thorough, full-body skin assessment] once a week.”

Clinical practice should relate to the subscore of moisture on the Braden Scale, says Cindy Sylvia, MSC, MA, RN, CWOCN, educational development program manager for Gaymar Industries.

It’s important to note that humidity—when coupled with high temperatures—can increase perspiration and raise the risk of problems such as fungal infection and skin degradation.

“We need to do our best to keep skin dry and be on alert for moisture-induced infections,” Goldman reasons, adding that moisture is commonly trapped in skin folds of the abdomen, breasts and groin areas of the body.
Of course, this doesn’t mean staff should forego moisturizers. On the contrary. Keeping skin properly hydrated is essential for maintaining skin integrity and creating a healthy moisture barrier.

“Skin moisturizers should be applied to skin at least once a day, no matter the season, especially if the nursing home resident has a history of dry skin,” Brangman says.

“Harsh soaps, like those found in the soap dispensers in many resident’s rooms, should be avoided and soaping generally should be focused on strategic areas, such as the genitals and underarms. Rinsing with hot water should be avoided. Rinsing thoroughly with lukewarm water should be considered, especially for those with a history of dry skin.”

Bottoms up

Oral fluid intake is also essential for maintaining skin health, particularly in warmer weather months when dehydration can occur rapidly and often with little warning.

“In the summer, it can get quite hot and muggy, and I always worry about dehydration,” adds Goldman. “This can be especially problematic for residents on water pills. We push fluids and even have a cooler on our patio to encourage fluid intake. You have to stay on top of it.”

Residents who are able to enjoy time outdoors should avoid the sun entirely during the hottest part of the day—typically between 10 a.m. and 3 p.m.—and should have sunscreen applied at least 20 minutes prior to venturing outside. It should then be reapplied as needed, depending upon their exposure time and rate of perspiration. Family members, who are typically responsible for supplying sun protection products, should also be informed of their importance and encouraged to keep the resident protected during any off-site trips.

Sunscreen always

Because thin, fragile skin is more susceptible to sunburn, sunscreen should be applied year-round when residents are heading outdoors, experts advise.

“I suggest one that has the physical blocks, titanium and zinc. Also, a hat in the summer is a must for added protection against sun damage,” says skin care expert Joanna Vargas of Joanna Vargas Skin Care in New York.
Goldman, too, suggests caregivers aim high on protection. He prefers a sunscreen with at least SPF 45, a wide-brimmed hat and lightweight, light colored moisture-wicking clothing.

He also urges skilled nursing providers to adequately train staff, especially certified nursing assistants (the most hands-on with residents) about the importance of sun protection, even on overcast days. Further, it’s essential that caregivers and family members understand that sun protection is important for all residents, even those with darker skin.

“It’s important not to forget to apply sun block to all potentially exposed areas. It’s common to focus attention on the face and arms, but any exposed skin is susceptible,” Goldman says, noting that the feet, for example, may be inadvertently overlooked.

Generally, those with a history of skin cancer should avoid the sun altogether. Specific instructions should be obtained from residents’ physician in these instances, according to Brangman.

Check meds

If vitamin D deficiency is a concern, vitamin D supplements are a viable option.

“The sun is a good source of vitamin D, but for those with a history of skin cancer [or other conditions that make them more susceptible to the sun] it’s better to exercise caution and avoid it altogether,” Goldman explains.

Skilled nursing residents’ medications should be carefully considered before taking residents outdoors during the warm, sunny summer months. A number of drugs could put residents at greater risk for sun sensitivity or dehydration, so caregivers should stay alert to any medication changes and make adjustments to outdoor visits, as necessary.

“Some medications, such as hydrochlorothiazide, an antihypertensive used to control elevated blood pressure, can cause photosensitivity or a rash after the skin is exposed to the sun,” according to Brangman.

“Nursing home residents should have a review of their medications done by a physician or pharmacist to see if they are taking any medications that might cause a skin reaction after sun exposure.”

Certain perfumes also can cause a photosensitive skin reaction when exposed to sunlight. Brangman suggests seniors avoid using perfume, cologne, aftershave or other scented products when planning to spend time outdoors.

Fragrance-free moisturizers and skin care products are another good idea and can help residents avoid allergic reactions and another common pitfall associated with spending time outdoors: insect bites.

“Doing what we can to prevent bug bites and other forms of skin irritation is just” good practice, Tully stresses.
He adds that persistent scratching can result in skin tears and even decubitus ulcers. Therefore, in regions where mosquitoes and other biting insects are prevalent, providers might want to consider using mild bug repellants, especially in the evenings and in more shaded, woody areas where insects tend to pose a greater threat.

“There’s a lot to consider, but with the proper assessments, good hydration and the use of good products and common sense, it is possible to keep skin healthy year-round,” Tully assures.
